Graphics Unit

FUNCTIONS OF THE GRAPHIC UNIT

The Unit is primarily charged with the following responsibilities:

  • Writing and issuance of Certificates to all the Nineteen (19) cadres of Registered Nurses and Midwives
  • Processing of Certificates and despatching of all signed Certificates to the various schools and individuals
  • Keeping records and statistics of all processed and dispatched Certificates
  • Ensuring the safety of processed, returned, unclaimed or blank certificates
  • Issuance of “To Whom It May Concern” in lieu of lost Certificate
  • Visualizing and Originating ideas for all artistic work of Council
  • Performing other Administrative duties as directed

CERTIFICATE POLICY

  • Certificates are issued once and cannot be reissued if lost
  • Certificates are not posted
  • Certificates are despatched directly to schools for Nigerian trained Nurses However, for Overseas trained Nurses/Midwives and all other Post Basic Programmes not conducted by the Council, their Certificates are despatched directly to them.

PROCEDURE FOR COLLECTION OF CERTIFICATES – OVERSEAS TRAINED/EDUCATORS AND ADMINISTRATORS

  • Handwritten Application
  • Original and photocopy of Notification of Registration
  • Original and photocopy of Testimonial/Certificate from school of training Original and photocopy of current practicing Licence
  • Submission of two recent identical coloured passport photographs
          - full face on white background, no eye-glasses
          - Size:– 5cm long x 4cm wide with names and registration numbers written at the back of the passports
          - Originals for sighting

PROCEDURE FOR COLLECTION OF CERTIFICATES FROM COUNCIL (LOCALLY TRAINED)

  • Letter of identification from School of training in the School’s letter head, signed and stamped by the Principal authorizing Council to dispatch certificate to owner
  • Original and photocopy of Notification of Registration
  • Original and photocopy of current practicing License
  • Submission of two recent identical coloured passport photographs
          - full face on white background, no eye-glasses
          - Size: 5cm long x 4cm wide with names and registration numbers written at the back of the passports
  • If you are sending someone on your behalf for collection:

  • A letter of authority
  • Valid identity card – (any of the followings: National ID Card, Driver’s Licence, International Passport or Current Nursing Licence). Along with items 1–4 above should be presented. Please note that third party collection is not automatic)

LOST CERTIFICATE – REQUIREMENT FOR ISSUANCE OF “TO WHOM IT MAY CONCERN” IN LIEU OF LOST CERTIFICATE

Certificates cannot be re-issued if lost and duplicate Certificates are not available. However, a “To Whom It May Concern in lieu of lost Certificate” may be issued after verification.

REQUIREMENTS

  • Letter of identification from School of training in the School’s letter Head, informing the Council of the loss
  • Original copy of sworn Affidavit stating details of lost Certificate and circumstance of loss
  • Photocopy of Certificate of Registration
  • Original and photocopy of Police report
  • Photocopy of current practicing License
  • Photocopy of Birth Certificate
  • Submission of two recent identical coloured passport photographs, full face in white background, no eyeglasses, with names and registration numbers written at the back of the passport and endorsed by the Principal
  • Payment of the prescribed fee of Eight Thousand Seven Hundred Naira (N8,750) only, per qualification into Nursing and Midwifery Council of Nigeria through REMITA
  • Handwritten application

Please note that the submission of the above requirements does not automatically guarantee, the issuance of “To whom it may concern”

REQUIREMENTS FOR RETRIEVAL OF UNCLAIMED CERTIFICATE

  • Handwritten Application
  • Original and Photocopy of Notification of Registration
  • Original and photocopy of Certificate from School of Training for sighting
  • Original and photocopy of Current Practicing License
  • Submission of two recent identical coloured passport photographs, full face on white background, no eyeglasses, with names and registration numbers written at the back of the passport
  • Payment of prescribed fees of Thirteen Thousand, Two Hundred and Fifty Naira (N13,250) per Certificate to Nursing and Midwifery Council of Nigeria through Remita Portal
